Depending on program needs, this role includes structured patient check-ins, coaching and goal-setting, care coordination to close gaps in care, and addressing barriers such as access to resources, transportation, or medication refills. Clinical Navigators document patient interactions in the Cadence platform and collaborate closely with the clinical team to escalate concerns and coordinate next steps, ensuring patients feel supported, informed, and engaged throughout their care journey.

• Reliable and consistent in schedule adherence, with the interpersonal communication skills to build trust with patients over the phone.
• Comfortable working independently in a remote, fast-paced environment with evolving workflows, and tech-savvy enough to navigate and troubleshoot multiple systems simultaneously while keeping patients engaged.
• Comfortable using AI-assisted clinical tools (such as Gemini or Open Evidence) to support patient interactions and surface relevant clinical context, while maintaining clinical judgment as the primary guide for every patient decision.
• Biased toward action - you bring clinical authority to every interaction, make the call when it counts, and take ownership of outcomes.
